Enrollment Paperwork

Congratulations, and welcome aboard United Communities AmeriCorps for your service journey in SW Oregon!

Before we are able to get started, there is a little administrative paperwork for us to take care of. Let our staff know of any questions you have!

1. Accept your offer in MyAmeriCorps!

Once you've accepted the site supervisor's offer to serve in the project you applied to, UCA will select you in the MyAmeriCorps online portal for service with us. At this point, you will receive an email notification from AmeriCorps stating that you've been selected for a position with United Communities and to log-in to your MyAmeriCorps account to accept the offer.

Be sure to do this before moving forward with other enrollment form items if you haven't already.

3. Complete our Enrollment Forms Packet (individual forms below).

There are two ways to complete these, either:

      • Download and complete each item electronically - e-signatures are required. Submit completed forms to UCA by email.

      • Print and complete physically - be sure to sign all items. Submit completed forms to UCA by mail or scanning and emails.

Members may not begin service prior to initiating background. Please submit this form as soon as possible to UCA.

AmeriCorps and the Corporation for National & Community Service are required to verify and document in your file that you are a U.S. citizen, National or Lawful Permanent resident. UCA is required to view an original and place a copy of member's 1) Social Security Card, 2) State Issued ID/Driver's License, and 3) One of the following in your member file:

    1. Primary documentation of status as U.S. citizen or national. The following are acceptable forms of certifying status as a United States citizen or national:

          • a birth certificate showing you wereborn in one of the 50 states, the District of Columbia, Puerto Rico, Guam, the U.S. Virgin Islands, American Samoa, or the Northern Mariana Islands;

          • an unexpired U.S. passport issued to an individual as a U.S. citizen;

          • report of Birth Abroad of a Citizen of the United States (U.S. Dept. of State Form FS-240);

          • certificate of birth—foreign service (U.S. Dept. of State Form FS-545);

          • certification of Report of Birth (U.S. Dept. of State Form DS-1350); INS certificate of naturalization (INS form N-550 or N-570); or

          • INS certificate of citizenship (INS form N-560 or N-561).

    2. Primary documentation of status as a lawful permanent resident alien of the U.S. The following are acceptable forms of certifying status as a lawful permanent resident alien of the United States.

          • Permanent Resident Card or Alien Registration Receipt Card (INS Form I-551) an unexpired passport indicating that the INS has approved it as temporary evidence of lawful admission for permanent residence; or

          • a departure record (INS Form I-94) indicating that the INS has approved it as temporary evidence of lawful admission for permanent residence.

UCAN distributes bi-monthly living allowances on the 7th & 22nd of each month. The first living allowance is always issued as a physical check via mail, but all others will be issued via direct deposit to this account.

The AmeriCorps living allowance is subject to federal taxes.

The AmeriCorps living allowance is also subject to Oregon State taxes.

This form is for verification with UCAN's HR & Fiscal department.

UCA will provide health care insurance to members who are serving full-time and who are not otherwise covered by a healthcare policy at the time the member begins their term of service.

UCA members who are already covered under another health insurance policy prior to enrollment as a AmeriCorps service member may not qualify for the UCA provided health insurance. UCA members who are not covered by another insurance plan prior to enrollment as a UCA member must apply to the Oregon Health Plan, which provides stronger overall coverage than the AmeriCorps Health Plan provides. Members who do not qualify for the Oregon Health Plan must provide proof they were denied this coverage before they will be enrolled in the UCA program’s sponsored plan.

UCA will provide healthcare insurance to members serving full-time who lose coverage during their term of service as a result of service or through no deliberate act of their own. Members who lose coverage during their term of service must immediately contact UCA Program Director to get enrolled in the AmeriCorps Health Plan, in order to assure the member is provided continuous ACA-compliant coverage throughout the member’s service year.

Members who are covered under other insurance (either the Oregon Health Plan or another third party provider) must complete a waiver form and provide a copy of their ACA-approved proof of insurance, such as a health insurance card, before being allowed to deny or be taken off of the insurance provided by UCA.

The UCA provided insurance is through the Corps Network with medical coverage underwritten by CIGNA and Accidental Death and Dismemberment (D&D) underwritten by Gerber. Additional dental/vision insurance is not provided. Family members are not eligible for medical coverage.
