End of Service Paperwork
Thank you for your service, here in SW Oregon!
Members must complete AmeriCorps exit paperwork before their final day of service and submit it United Communities. Exit paperwork may take a bit of time, so do not wait until your last day. Use the items below to guide you through the process.
1. Finalized Monthly Paperwork
Timesheets:
Ensure all timesheets are submitted & approved by site supervisor
Monthly Report Form:
Email to UCA Staff
cc’ your site supervisor
Title subject line: “Final MRF: YOUR NAME”
Teleservice Report Form (if applicable)
Email to UCA Staff

2. Exit Forms
Download and complete this form in Microsoft Word (.doc format) and submit to UCA staff by email

MyAmeriCorps: Exit Form
Complete this in MyAmeriCorps, which can be accessed under the "Close of Service Forms" heading.

Complete this in MyAmeriCorps, which can be accessed under the "Close of Service Forms" heading.
3. Supervisor Exit Forms
Members are responsible for submitting the following two forms which their Site Supervisor must complete alongside the rest of their Member Exit Paperwork. Members should check-in with their supervisors regarding these items at least two weeks prior to their exit date.
